In registering as a player or member of the Club I hereby agree to indemnify the Club against any loss or damage which I may suffer or incur in relation to activities as a player or member or as the result of actions of the Club. This indemnity is given and granted by the player or member. Within the subscription the Club provides a Limited Sports Injury Cover as promoted by HockeyWA and underwritten by Sportscover Australia. PREGNANCY STATEMENT The following wording is derived from advice provided by Minter Ellison Sports and Litigation Lawyers.

"Pregnant players are advised the continued playing during pregnancy exposes them and their unborn child to health risks. Advice should be sought from an appropriately qualified medical practitioner about those risks and whether, or for how long, it is safe to continue participating.

Australian Law imposes on a pregnant player a duty to take reasonable care to avoid foreseeable risks of injury to her unborn child. If a player fails to do so, she runs the risk of being sued by her child later in life.

Players owe no greater duty of care to a pregnant player than they owe to another player. Players are liable only for those injuries that they cause by acting outside the rules of the game in a manner that is not ordinarily and reasonable contemplated by other players.

The duty of care owed to an unborn child by another player is similar to that owed to the mother and all other players; namely, to play by the rules in a manner that other players would ordinarily and reasonably expect."

I AGREE TO ABIDE BY THE FOLLOWING CLUB POLICIES:

  1. Coaches, Managers and Committee members may have players contact details provided to them. The information is used for the day to day running of the Club, including maintaining HockeyWA HockeyNet records and providing hockey information to players.
  2. The playing season consists of a set of regular fixtures that may INCLUDE 2 ROUNDS OF UMPIRING DUTIES. Failure to perform these duties when required to do so will result in the Club incurring a fine from HockeyWA, which will be passed on to the member concerned. Together with this, each team will be rostered to assist with the management of the bar during the season.
  3. The playing uniform is for sale to players only from Hockey Action in Victoria Park. All players must wear the official Club uniform.
  4. Subscriptions must be paid by the 10th May 2010 unless an arrangement has been made with the Finance Convenor. Beyond this date there is no entitlement to bar vouchers and players may be ineligible to play at the Committee's discretion.
  5. To receive the full time student discount the player must provide their student number & if requested their student card
